McDaniel College Professional Support Overview Request Form

A Professional Support Overview Form is required for all Elevate applicants.

To initiate the request, please complete the form below. Once complete, McDaniel College will send an email with a secure link to the Professional Support Overview Form to the provider you designate. The professional/provider must be currently working with the applicant. Applicants are strongly encouraged to notify the designated provider that the Provider Overview Form will be sent.

The Professional Support Overview Form includes a thorough assessment of the applicant’s academic, behavioral, and social performance.

Your application for Elevate will not be considered complete until your Professional Support Overview Form is received.
